A Lincoln therapist faces four felony charges in an alleged Medicaid billing scam.

Court records say Stella Offordirinwa is accused of two counts of theft by unlawful taking and two counts of attempting to obtain payment for medical assistance by submitting a false statement.

Court records say the Nebraska Medical Assistance Program has terminated her five-member firm as a service provider.

An investigator said in an affidavit that Offordirinwa billed Medicaid under other therapists' names for work they didn't do and billed Medicaid twice for denied claims.

The affidavit said the claims were submitted between March 2005 and Aug. 12, 2007.

Efforts by The Associated Press to reach Offordirinwa on Thursday were unsuccessful.
